This policy guide provides guidelines for family unit case name rules, as well as the procedure for resolving issues with family unit case names or State ID numbers.

POLICY

Family Unit Case Names

The family unit case name provides a uniform basis for assignment of other case-identifying information, such as state numbers and assistant unit numbers.  The correct assignment of family unit case names and State Identification numbers prevents duplication of case openings. 

The SAAMS Unit is responsible for determining the family unit case name,  the state number of a case, and assistant unit numbers.  The 19-digit case number is assigned by CWS/CMS.  The SAAMS unit also addresses any corrections, duplications, or related problems, and may request CSW input and verification of information as needed.

Assigning Case Names

The following table indicates the case names that will be used in a given situation:

Child in home of father – mother is absent and her absence is expected to be permanent

Child in out-of-home care and mother’s State ID is known

Child with legal guardian

  • Mother (if known)

  • Child (if parents’ names are unknown)

Child in out-of-home care and the mother’s name is unknown

Child

Dependent infant placed with dependent minor mother in out-of-home care

Mother of minor mother (infant’s grandmother)

Non-dependent infant placed with dependent minor mother in out-of-home care

Mother of minor mother (infant’s grandmother)

Parental rights terminated

PROCEDURE

CSW Responsibilities

  1. Submit a DCFS 264, SAAMS Data Inquiry & Action Request Form, to the SAAMS Unit, describing the problem.
  2. Document in CWS/CMS Case Notes:
    1. Family Unit name/state number
    2. The problem encountered
    3. Date DCFS 264 was submitted
