Overview

This procedural guide provides instruction on procedures for an address change when the child/youth or nonminor dependent (NMD) is in a placement under the Foster Care (FC), Foster Care Probation, or Kinship Guardianship Assistance Payment (Kin-GAP) Programs.

This policy guide was updated from the 11/03/2014 version to reflect current Eligibility Worker (EW) procedures and align them with the implementation of the California Statewide Automated Welfare System (CalSAWS).

POLICY

Payment Resolution Unit (PRU)

The Revenue Enhancement Division (RED) Payment Resolution Unit (PRU) Eligibility Worker (EW) and Foster Care (FC) or Kinship Guardianship Assistance Payment (Kin-GAP) Intake/Redetermination EW have specific tasks that must be completed and/or followed up with to ensure the timely and accurate processing of an address change when a child/youth or NMD is placed in a:

  • Group Home (GH)/Short-Term Residential Therapeutic Program (STRTP)
  • Foster Family Agency Resource Family Home (FFARFH)
  • Supervised Independent Living Placement (SILP)
  • Resource Family Home (RFH) (which includes relatives, nonrelated extended family members [NREFMs], relative legal guardians [RLGs], or non-related legal guardians [NRLGs].

Address changes for Adoption Assistance Program (AAP) are completed by the AAP PRU. The timely and accurate processing of address changes prevents or minimizes overpayment/underpayment issues and funding errors while ensuring ongoing payment to meet the needs of the child(ren) placed in a facility or home.

Requirements for an Address Change

All requested address change actions are assigned to a PRU EW. Requests for an address change require either a DCFS 280 or RM 200 which authorizes the address change and validates the action taken by Eligibility staff. Address changes for FC Probation cases require a PROB 667 to be submitted to the FC Probation EW from the Probation Department. The EW will then submit the RM 200 to the PRU. Address Change Notifications

Revenue Enhancement may receive notice of an address change or potential address change in the following manner:

  • Automated DCFS 280, Technical Assistant Action Request
  • PROB 667, Probation Department Placement Authorization
  • Phone call to Foster Care Hotline (FCH) at (800) 697-4444
  • Phone call to Caregiver Warm Line at (877) 323-7165
  • Email to PRU Inbox PRU@dcfs.lacounty.gov (with applicable attachments (e.g., RM 200))
    • On every address change request received, a Work Order will be created indicating the requested action. Upon completion of the assignment, the EW who completed the assignment will update the Work Order noting the action taken.

Types of Address Changes

The following types of address changes may be received by DCFS:

  • Resource Family Home (RFH) including but not limited to: relative caregiver, NREFM, or non-relative non-guardian
  • Legal Guardian (related or non-related)
  • SILP
  • Different resource family home within the same FFA
    • When the FFA certified home has had an address change but the child/youth remains with the same caregiver, the FFA completes the address change.
  • Different Group home/STRTP site within the same GH/STRTP agency
  • Different Transitional Housing Placement Program (THPP) site within the same agency

Due to authority limitations in CWS/CMS and California Statewide Automated Welfare System (CalSAWs), coordination with the San Dimas Resource Management Unit (RMU) is needed to complete some of the address change functions.

The PRU EW is required to notify the Children’s Social Worker (CSW) and the Intake/Redetermination EW via e-mail upon completion of the address change.

Foster Family Agency (FFA)/Group Home (GH)/Short-Term Residential Therapeutic Program (STRTP) Placement

A child/youth placed in an FFA and moved from one (1) certified home to another, or a child placed with a GH/STRTP is moved from one facility to another is processed as a “replacement” on CWS/CMS, however it is an “address change” for the child.  These circumstances do not require new placement packets or Medical Authorization letters, but there may be times when the CSW will request one or the other. The TA/EW will provide the documents if requested by the CSW. Payment and Medi-Cal continues to be issued to the FFA or GH/STRTP umbrella agency.

Supervised Independent Living Placement (SILP) Services

According to the Re-Entry into Extended Foster Care (EFC) policy, when an NMD moves from a regular foster care placement to an SILP or from one (1) SILP to another SILP, SOC 157A and SOC 157B forms are required before budgeting and address changes can be completed.

Out-of-County/State Address Changes

If the caregiver’s new address is outside the County of Los Angeles, see Foster Care Placement/Replacement, Inter-county Transfer, Interstate Compact on the Placement of Children (ICPC), and General Relief Ineligible (GRI) to AFDC-FC policies for further instructions on funding the placement

Returned Warrant from Auditor-Controller (A-C)

The Audit-Controller’s (A-C’s) office receives returned warrants and/or remittance stubs, often due to an address change not having been reported to the Department of Children and Family Services (DCFS). The United States Postal Service (USPS) has a policy that mail (including warrants) sent out from the A-C will not be forwarded to a new address but will be returned to them by the USPS. The USPS will indicate on the outside of the envelope the disposition such as “addressee unknown” or “return to sender” indicating that they were unable to deliver the mail to the addressee.

The A-C will cancel the outstanding warrant. Once that is completed, CalSAWS automatically suspends the vendor number of the caregiver until confirmation is received regarding the correct address. Upon an address change or confirmation that the current address is correct, DCFS PRU will be required to verify the vendor address information and lift the suspension to reissue the cancelled warrant.

This does not reflect information that the banking institution has on file for the caregiver, only the address information the DCFS has been provided by the caregiver.

PROCEDURE

Regional Staff Receive a Request for an Address Change

Technical Assistant (TA)/Eligibility Worker (EW) Responsibilities

  1. Email the Automated DCFS 280 requesting an address change to the PRU email inbox: PRU@dcfs.lacounty.gov and follow guidance provided in Foster Care Placement policy. 

Probation EW Notified of an Address Change

Probation EW Responsibilities

Upon notification of an address change via PROB 667 from the Probation Placement Unit (PPU) or Deputy Probation Officer (DPO), or email from PRU EW:

  1. Access the CWS/CMS application and open the existing case.
  2. Record the secondary assignment.
  3. Update CWS/CMS Case Notes that an address change was sent to the PRU for processing and save to database.
  4. Complete and forward the RM 200 to the PRU email inbox: PRU@dcfs.lacounty.gov.

Payment Resolution Unit (PRU) Unit Clerk (UC) Responsibilities

  1. Access the PRU Inbox.
  2. Review the RM 200 and any accompanying information to determine the type of assignment.
  3. Confirm assignment with PRU ES.
  4. Create a Work Order.
  5. Distribute the RM 200 to assigned PRU EW for processing of the address change.

PRU Eligibility Worker (EW) Responsibilities

  1. Review the RM 200.
  2. Access the CWS/CMS application and open the existing case to confirm completion of the address on the placement ID page, the Substitute Care Provider page, and the LA Payee page.
    • If not completed, PRU EW will update.
  3. Complete CWS/CMS Case Notes and save to database.
  4. Access case in CalSAWs to update address in Contact Summary page and Foster Care Resource Detail page.
  5. Update the Work Order and mark it as completed.
  6. Email the Probation EW, the MEDs unit via inbox: MEDSreferrals@dcfs.lacounty.gov and PRU ES confirmation of the completion of the address change.
  7. Image all documents into CalSAWS.

PRU Eligibility Supervisor (ES) Responsibilities

  1. Review the completed address change documentation upon receipt.
  2. For address changes within an agency, access CWS/CMS to approve Ongoing Requests page and Placement page and save to database.

Escalation Process

PRU EW Responsibilities

  1. Contact the case-carrying CSW or Probation EW to request the necessary documentation.
    1. If there is no response after two (2) attempts or within six (6) business days, inform the PRU ES.

PRU ES Responsibilities

  1. Contact the Supervising Children’s Social Worker (SCSW) or Probation ES to request the necessary documentation.
    1. If there is no response after two (2) attempts or within six (6) business days, the ES will escalate the request to the Human Services Administrator I (HSA I) to inquire about the information/documentation needed from the Assistant Regional Administrator (ARA), Regional Administrator (RA), and/or Probation HSA I.

Resource Family Home (RFH), Legal Guardian (LG) Home, or Supervised Independent Living Placement (SILP) Address Change

As a result of an address change for a RFH, LGH, or SILP, it may be necessary for the Case-Carrying Eligibility Worker (CCEW) to update the Aid Code for the placement and/or issue a Notice of Action (NOA). Please refer to Redetermination of AFDC-FC Eligibility policy for more information. Additionally, actions may be required for the Medi-Cal Eligibility Worker (EW); please refer to Medi-Cal Benefits policy for more information.

Foster Care Hotline (FCH) or Warm Line Responsibilities

  1. Receive a phone call indicating that an address change is required where the caregiver has moved and the child/youth remains placed with the same caregiver.
  2. Complete a Work Order, assigning the task to an EW in the PRU Unit.

PRU EW Responsibilities

  1. Receive and review the Work Order to ensure mandatory fields/information is available.
  2. Contact the caller via telephone to verify the address.
    • If the caller is the CSW, skip to step 4.
  3. Contact the CSW via email to notify them of the address change and request confirmation of address.
  4. Update only the mailing address.
  5. Request the CSW to submit an Automated DCFS 280 to update the physical address.
  6. Once received, review the DCFS 280 to ensure mandatory fields are completed and the form has appropriate signature authorization.
    1. If DCFS 280 is missing information and/or signatures, contact the CSW and request a corrected DCFS 280.
  7. Access the CWS/CMS application and open the existing case.
  8. Record the secondary assignment.
  9. Review and update the placement ID page, the Substitute Care Provider page, and the LA Payee page.
  10. Record all activity in the CWS/CMS Case Notes.
  11. End date the secondary assignment and save to database.
  12. Access case in CalSAWs to update address in Contact Summary page and Foster Care Resource Detail page.
  13. Email the CSW, SCSW, CCEW, Case-Carrying Eligibility Supervisor (CCES), the MEDs unit via inbox: MEDSreferrals@dcfs.lacounty.gov, and PRU ES confirmation of the completion of the address change.
  14. Image all documents into CalSAWS.

PRU ES Responsibilities

  1. Receive and review completed address change documentation.
  2. For address changes within an agency, access CWS/CMS to approve Ongoing Requests page and Placement page and saves to database.

Replacement Within the Same Foster Family Agency (FFA) (Child is Placed with a New Different Caregiver)

PRU UC Responsibilities

  1. Receive Automated DCFS 280 via email indicating that a child/youth has moved to a different FFA Resource Family Home (remaining in the same FFA).
  2. Confirm assignment with PRU ES.
  3. Create a Work Order for PRU EW.
  4. Forward the DCFS 280 and any additional attachments to the assigned PRU EW.

PRU EW Responsibilities

  1. Receive and review the DCFS 280 to ensure mandatory fields are completed and the form has appropriate signature authorization.
    1. If DCFS 280 is missing information and/or signatures, contact the CSW and request a corrected DCFS 280.
  2. Access the CWS/CMS application and open the existing case.
  3. Record the secondary assignment.
  4. Search for the FFA Resource Family Home (RFH).
  5. Review the placement ID page, the Substitute Care Provider page, and the LA Payee page.
    • If the FFA RFH address change has already been recorded, indicate assignment complete on the Work Order.
    • If the FFA RFH address change has not been completed, complete a replacement to the new FFA RFH on CWS/CMS.
  6. Record all activity in CWS/CMS Case Notes.
  7. End date the secondary assignment and save to database.
  8. Access case in CalSAWs to update address in Contact Summary page.
  9. Email the CSW, SCSW, CCEW, CCES, the MEDs unit via inbox: MEDSreferrals@dcfs.lacounty.gov, and PRU ES confirmation of the completion of the address change.
  10. Image all documents into CalSAWS.

PRU ES Responsibilities

  1. Receive and review completed address change documentation.
  2. Access CWS/CMS to approve Ongoing Requests page and Placement page and save to database.

Replacement for Transitional Housing Placement (THP)- Nonminor Dependent (NMD) Program (Moves to a Different Address Within the Same Agency)

PRU UC Responsibilities

  1. Receive Automated DCFS 280, Certificate of Compliance via email indicating that a NMD has moved to a different address with the same THP agency.
  2. Confirm assignment with PRU ES.
  3. Create a Work Order for PRU EW.
  4. Forward the Automated DCFS 280 and attachments to the assigned PRU EW.

PRU EW Responsibilities

  1. Receive and review the Automated DCFS 280 and attachments to ensure mandatory fields are completed and the request has appropriate authorization.
    1. If Automated DCFS 280 is missing information and/or approvals, contact the CSW and request a correction.
  2. Access the CWS/CMS application and open the existing case.
  3. Record the secondary assignment.
  4. Search for the THP.
  5. Review the Placement ID page, the Substitute Care Provider page, and the LA Payee page.
  6. Complete a replacement to the new address on CWS/CMS.
  7. Complete address change steps.

PRU ES Responsibilities

  1. Receive and review completed address change documentation.
  2. Access CWS/CMS to approve Ongoing requests page and Placement page and saves to database.

Replacement within the same Group Home (GH)/Short-Term Residential Therapeutic Program (STRTP)

FCH Clerk Responsibilities

  1. Receive notification that an address change is required for placement from one site to another under the same GH/STRTP.
  2. Confirm assignment with PRU ES.
  3. Complete a FCH Work Order, assigning the task to an EW in the San Dimas PRU.
  4. Forward the Work Order to the PRU EW.

PRU EW Responsibilities

  1. Receive and review the Automated DCFS 280 to ensure mandatory fields are completed and the form has appropriate authorization.
    1. If Automated DCFS 280 is missing information and/or signatures, contact the CSW and request a correction.
  2. Access the CWS/CMS application and opens the existing case.
  3. Record the secondary assignment.
  4. Review the placement information including the address page.
    • If the address change (change of facility site) has already been recorded, indicate assignment complete on the Work Order.
    • If the address change (change of facility site) has not been completed, process in the same manner as a group home replacement (see Foster Care Placement.) Keep in mind that there may be a change in the rate amount with a change in facility.
  5. Complete address change steps.

PRU ES Responsibilities

  1. Receive and review completed address change documentation.
  2. Access CWS/CMS to approve Ongoing Requests page and Placement page and save to database.

Auditor-Controller (A-C) Forwards DCFS the Direct Deposit Warrant /Remittance Stubs

PRU UC Responsibilities

  1. Receive direct deposit warrant/remittance stubs from the A-C.
  2. Check for an existing Work Order.
    1. If there is no existing Work Order, verify assignment with PRU ES and initiate a new Work Order, assigning the task to a PRU EW.
  3. Forward the Work Order, remittance stub and the return envelope to the assigned PRU EW.

PRU EW Responsibilities

  1. Receive a Work Order from the PRU UC along with any attached documentation advising DCFS that direct deposit statements (i.e. remittance stubs) have been returned.
  2. Access the CWS/CMS application and open the existing case.
  3. Record the secondary assignment.
  4. Follow the verification of address and address change steps.

PRU ES Responsibilities

  1. Receive and review completed address change documentation.
  2. Access CWS/CMS to approve Ongoing Requests page and Placement page and save to database (if applicable).
